Thank you for accepting the invitation to participate in this project. The following surveys will be used as part of a research project into different thinking styles and how people process mental tasks. You have been invited to participate because the more people that are involved the more accurate the results should be.
The questions should take you about 30 minutes to answer. The results obtained will be both confidential and anonymous. You may stop at any time, and all your data will be discarded. If you would like to withdraw after the survey is complete, please email your participant number (it will be at the top of every page of the survey) to Sabrina Zitzelberger at firstname.lastname@example.org, and your data will be discarded. Your participation is completely voluntary and failure to participate will not penalize you in any way.
This research will involve you taking a survey and answering some basic math questions. The risks associated with this pose no risks greater than those you normally face in your day-to-day life.
This research will allow us to understand more about the ways different people approach problems. It could lead to additional research that might impact educational and other psychological studies.
This form will be stored separately from the data collected, but all information received will be kept confidential and stored in a secure location accessible only by the researcher and her instructor. No data will be tied to names or other identifying information--each survey and test will only be identified by number. No individual data will be presented.
